The Peninsula Region is seeking passionate and dedicated individuals to join our team as Community Health Workers. We are looking for individuals who are committed to improving the health and well-being of our diverse community.

What You'll Do:

  • Connect with individuals and families: Build trust and rapport with community members, providing culturally sensitive and empathetic support.

  • Navigate complex systems: Help individuals access essential resources, including healthcare, social services, and community programs.

  • Promote health and wellness: Educate community members about health issues, disease prevention, and healthy lifestyle choices.

  • Advocate for change: Identify community needs and advocate for policies and programs that improve health equity.

  • Collaborate with a team:Work closely with healthcare providers, community organizations, and other stakeholders to achieve shared goals.

Qualifications

One year of healthcare and/or community-based work experience that would demonstrate attainment of the requisite job knowledge/abilities. Post-secondary community health worker training is preferred.

Certification/licensure as an MA, CNA, HHP or LPN is preferred.

Pay Range

$19.73 - $27.13 /hour
