Comprehensive Care

Dominican offers emergency services and is a Certified Stroke Center and Chest Pain Center. The hospital’s heart attack treatment times consistently beat the national average, and the hospital offers two cardiac catheterization labs available 24 hours a day, 7 days a week. Groundbreaking cardiac procedures at Dominican include the Transcatheter Aortic Valve Replacement (TAVR) procedure, which allows for heart valve replacement without opening a patient’s chest.

Dominican Hospital has forged significant partnerships with area hospitals to bring exceptional care to Santa Cruz County. The hospital offers a level III Neonatal Intensive Care Unit (NICU) through a partnership with Lucile Packard Children’s Hospital Stanford. Neurosurgery at Dominican is also offered through a partnership with Stanford Health Care.

Responsibilities

The RN Care Coordinator is responsible for assessing planning facilitating and advocating for options and services through a continuum of care from admission to discharge for assigned patients. The RN Care Coordinator performs the above in such a manner as to meet the individuals health needs while promoting quality and cost-effective outcomes.

The positions emphasis will be on care coordination communication and collaboration with nursing physicians departments within the medical center insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care.

The RN Care Coordinator will advocate for the patient and family by identifying and valuing patient choice spiritual needs cultural language and socioeconomic barriers to care transistions. In addtion the RN Care Coordinator will will protect confidentiality while striving to achieve high levels of patient satisfaction. The RN Care Coordinator consistently conducts the utilization review process and discharge planning process per hospital policies standards of practice and by Federal and State Regulations.

Qualifications

Experience:

  • Minimum of Three years nursing experience in an acute hospital setting

  • Prior Care Coordination experience in aclinical or insurance setting. If operational conditions permit training a candidate without the required experience may be considered.

  • Working knowledge of the RN scope of practice current state requirements CMS Conditions of Participation EMTALA Patient Bill of Rights AB 1203 and other federal or state regulatory requirements as specific to Utilization Review and Discharge Planning.

  • Fluent in English verbal and written skillsWorking knowledge of Microsoft Office and work related software

Licensure:

  • Current and active CA RN license

  • Current BLS certification

Preferred:

  • One year case management in an acute care hospital

  • Bachelors degree (current incumbents are exempt from this criteria.

  • Masters degree in nursing with a concentration in Case Management may serve as a substitute for the experience requirement.

  • Certification appropriate for the position (CCM ABQAURP or ACM)Bilingual skills

Pay Range

$93.20 - $98.79 /hour
