Responsibilities

Community Health Workers (CHW) are essential frontline public health professionals who possess a deep understanding of and trust within the communities they serve. This unique relationship enables CHWs to act as liaisons between health and social services and the community, thereby facilitating access to services and enhancing the quality and cultural relevance of service delivery.

CHWs play a critical role in empowering individuals and communities by increasing health knowledge and promoting self-sufficiency through activities such as outreach, community education, informal counseling, social support, and advocacy. They serve as vital connectors between the community and various health care, government, and social service systems.

This 480-hour internship offers supervised on-the-job training with a certified Community Health Worker, requiring a commitment of 10-15 hours per week.

  • Assist individuals and communities in adopting healthy behaviors.

  • Conduct outreach for medical personnel or health organizations to implement programs that promote, maintain, and improve individual and community health.

  • Provide information on available resources, social support, and informal counseling; advocate for individuals and community health needs; offer services such as blood pressure screening.

  • Conduct home health visits for chronically ill patients, pregnant women, nursing mothers, individuals at high risk of health problems, the elderly, and other at-risk populations.

  • Deliver health information using culturally appropriate terms and concepts.

  • Assist with administrative and program support for the CHW home visiting model (Pathways to Healthier Living Program).

  • Advocate for local health needs.

  • Collect data to help identify community health needs.

  • Establish and accurately maintain files, databases, and records.

*This position pays $20 per hour*

* POSITION WILL BE FOR 1 YEAR AND APPLICANTS WILL BE LIMITED TO ASU CHW PROGRAM COHORTS *

Qualifications

Minimum:

High School Diploma/GED or Official Transcripts

Preferred:

Bachelors Degree

Pay Range

$20.00 - $20.00 /hour
